DPHSS Issues Form SA-01: Declaration of Individuals Attesting to COVID-19 Vaccination
The Department of Public Health and Social Services (DPHSS) issued Form SA-01, relative to declaration of individuals attesting to COVID-19 vaccination to enter establishments, in accordance with Section B, Item 2(a)(i) of DPHSS Guidance Memo 2021-21. CLICK HERE to download Form SA-01. Under DPHSS Guidance Memo 2021-21, covered establishments may accept self-attestation as proof of vaccination. Covered establishments must keep self-attestation forms on file for a period of thirty (30) days and present them to DPHSS upon request.
